Notification Forms

The following forms can be used by law practices to give information to the Law Society as prescribed by the Legal Profession Act 2007 and the Legal Profession Regulation 2007:

QLS Form 01 (LPR): Notification by Interstate Legal Practitioner of Authority to Withdraw Money from Local Trust Account
QLS Form 02 (LPR): Notification by Interstate Legal Practitioner of Termination of Authorisation or Variation of Circumstances Relating to Authority to Withdraw Money from Local Trust Account
QLS Form 04 (LPR): Law Practice Declaration and Trust Money Statement
QLS Form 05 (LPR): External Examiner's Report
Non-Required Form: Notification provided by Law Practice - cessation of appointment as external examiner
Non-Required Form: Notification authorising or termination of signatories to general trust account
QLS Form 31 (LPA): Notice of Appointment or Replacement of External Examiner
QLS Form 32 (LPA): Notification provided by External Examiner of Ceasing to be External Examiner
QLS Form 33 (LPA): Statutory Declaration - Ceasing to Hold Trust Money
